Release: I, the undersigned, do hereby give Sambuca nightclub permission to use my likeness (picture, video, name, etc.) in advertising, promotions or video. I understand that these pictures will become the property of Zachary's and that no monetary fees or royalties will be paid to me or anyone representing me for the use of these likenesses. I also give my permission to the other parties involved in Zachary's Open Dance Competition (sponsors, representatives, etc) the right to use my likeness.

Long Island Dance Championships Rules and Regulations
1. Competition is open to contestants over 18 years of age.
2. There is no fee to enter.
3. Any partner dance style is acceptable.
4. One qualifying round is left. We are accepting 6 couples and top 2 couples will qualify into the semi finals.
5. Dancers will be judged in the following categories: difficulty of routine, technique, style, appearance, and stage presence.
6. Preliminaries will be Nov 14th. We will have three weeks of semi-finals (Nov 21st, Nov 28th, and Dec 5th) and finals to be held December 12th, 2009.
7. Weekly winners will receive $500 in cash and prizes. Finalist awards total $5,000 in cash and prizes.
8. Contestants are requested to bring with them the music they want to dance to (limited to 1 song per CD up to 4 mins max).
9. All contestants must fill out an application and waiver before they will be allowed to compete.
10. Competition begins at 10pm sharp! Pre-registered contestants must confirm their entry with the dance coordinator by 8pm on the night they are scheduled to compete or they risk their position being forfeited for that evening.
